[ ] Record the date the notice arrived. [ ] Record whether the decision was final, conditional, or a change to an existing account. [ ] For a credit decision, optionally record the affected application type: credit card, auto financing, mortgage, or other personal credit. SEPARATE THE THREE ROLES [ ] Decision-maker: the lender, creditor, landlord, employer, insurer, bank, utility, or provider that made the decision. [ ] Reporting company: a separate credit bureau or specialty consumer reporting company listed with an address and phone number. [ ] Score factors: statements explaining why a particular credit score was not higher; these may differ from the decision-maker's reasons. SOURCE CHECK [ ] Request the consumer report from the company actually named in the notice, if one is named. [ ] Ask the decision-maker which report or other source it used if the notice is unclear. [ ] Compare the report, application, and original records field by field. [ ] Classify what you found: inaccurate, accurate but unfavorable, possible identity theft, or still unclear. TIMING [ ] Check the notice for the deadline to request specific creditor reasons. [ ] Check the notice for the period to request a free source report. [ ] Track a later report dispute and its response separately.
